Medical Records-Health Information Management
The Medical Records department is responsible for maintaining an accurate medical record for each patient of Hancock County Memorial Hospital and it’s clinics. Medical records are created and stored in both paper and electronic form.
State and federal laws (HIPAA) govern access, use and release of medical information. To better understand how HCMH uses your information, please review our Privacy Notice. This notice is available in paper format at all our admission locations. As a patient, you will be asked to review this document and sign an acknowledgement of receipt upon admission or physician visit at our hospital or clinics (effective April 14, 2003).
As stated in our Privacy Notice [ English (328 KB) / Spanish (321 KB) ], you have the right to request copies of your medical record (some exceptions apply, as governed by State and Federal laws). To obtain personal copies of your medical record, you will be required to fill out a Patient Release Authorization Form (249 KB). You may call or stop at the hospital or any HCMH Clinic to obtain this form. Charges for personal copies will apply. Transfer of specific records for treatment purposes will be processed without charge (exceptions may apply).

Health records are available through the HCHS Portal. You will be invited at your appointment or during a hospital stay if you want to view your medical records through HCHS's secure portal. If you don't have an upcoming appointment and you have been seen since August 3, 2015, please call your HCHS Healthcare Provider by calling: 641-843-5050, The link below will take you to the enrollment site.
